Completion and submittal of this application does not constitute automatic acceptance into the mentoring program. Applicants will also interview, pass a background check, and references will be contacted before a final determination is made. Mandatory training for mentors takes place prior to pairing and meeting with a mentee.

A mentor's continued participation in the program is contingent upon regular attendance, positive attitudes and proper behaviors and may be revoked by Friends of Westchase or Paul Revere Middle School at anytime if deemed necessary.
